To prepare for your visit, make a list of your questions, so you won't forget!

Bring along your Immunization card and any recent reports or medical information.

Address changes

Don't forget to update us on new addresses, phone changes and e-mail changes. If you have Insurance, let us know.

If you cannot make the appointment, please be courteous and call.

For appointments :

Call (868) 645-9605 during office hours.

• Mondays to Fridays: 8am - 4pm
• Thursdays: 8am - 12 noon
• Saturdays: 9am - 12 noon

A word about contacting me by Pager:

I respond to pages sent to me depending on the urgency by which I assess the message. It is important that messages are sent as follows:

• State YOUR name first
• State your CHILD'S first and last name and age
• A brief message as to why you are calling

An example of such a call is shown below:

• "Please call Sharon, re Tanya Peters, age 2 months, with high fever"
• Pages which I do not consider to be urgent will be answered on the following day.
